Create online slam book

Hi, guys create your own slam book without paying money.

Required : enough one gamil account and basic html knowledge.

Recipe :
1. open this url docs.google.com in your browser to create a slambook form in html.
2. click on create button which appears at left side.
3. choose Form options in that dropdown menu.
4. enter title and select one template then click ok.

5. delete default question in the form.
















6. click on add question button in the bottom, there so many html fields available like text,check box,text area.... chose your fields as your requirement like the following.

7. click the send form button on botom.




















8. if your friends  have filled the details in your slambook form, it'll automatically stored into your spread sheet.
9. how to see,  open https://drive.google.com click on your spreadsheet(responses).
